Accountability has become one of those values that organizations talk a lot about and check a little. It appears in job descriptions, in objectives, in evaluations, and in leadership discourse. Yet when a problem arises, the question “who is responsible for this?” is often followed by a very long silence. Not because people don’t want to take responsibility, but because, often, they don’t know exactly what they need to take responsibility for.
